Topical Clascoterone Reduces Sebaceous Gland Count in Papulopustular Rosacea Patients

A Phase 2 trial shows twice-daily clascoterone cream 1% improves both clinical markers and patient quality of life.

A Phase 2 clinical trial published on August 1, 2026, found that clascoterone cream 1% applied twice daily over 12 weeks significantly reduces the mean sebaceous gland count in adults with papulopustular rosacea. For medical aesthetics clinics, this topical androgen receptor inhibitor offers a highly tolerable alternative targeting the hormonal and lipid-producing pathways of rosacea, which are traditionally difficult to manage with standard anti-inflammatory or antimicrobial topicals.

The study evaluated 20 participants, 18 of whom completed the full 12-week course. The mean sebaceous gland count dropped from 4.94 ± 1.98 to 3.61 ± 2.33 (P=0.012), showing a direct impact on the gland activity that drives inflammatory lesions. While the decrease in mean gland diameter did not reach statistical significance (P=0.182), patient-reported quality of life was notably improved. The mean Dermatology Life Quality Index score dropped from 7.5 ± 5.8 to 3.3 ± 5.6 (P=0.029). Patient-reported outcome scores showed positive responses for papules and pustules at 3.2 ± 1.1, facial redness at 2.7 ± 1.0, general skin appearance at 2.7 ± 1.1, and overall treatment satisfaction at 3.1 ± 0.7.

From a clinical safety standpoint, the topical was well tolerated. The trial recorded only two mild, self-resolving adverse events, which were one instance of transient dryness and one of mild eye irritation. No serious side effects occurred. If your clinic routinely treats stubborn rosacea patients who struggle with the side effects of systemic therapies, keeping watch on clascoterone as it moves through the pipeline provides a useful, patient-satisfying alternative to discuss during treatment planning.
